Thomas F. Lewis, Jr. was born in Freeport , Illinois , on September 8, 1938. Mr. Lewis received his Bachelors degree in 1960 from St. Joseph 's College. He received his Juris Doctorate in 1963 from the Indiana University School of Law. While in law school he was St. Joseph 's College, B.S., 1960: Indiana University , J. D., 1963 (Order of the Coif, Phi Delta Phi and Law Journal). He began practicing law in South Bend , Indiana in 1963. Member: St. Joseph County and Indiana State Bar Associations; American Health Lawyers Association; Indiana Association of Hospital Attorneys (Past Chairman and Lecturer). Former affiliations include: Board of Directors: Commercial Law Affiliates. Chamber of Commerce of St. Joseph County (Executive Committee 1998 - 1999); Healthy Communities Initiative of St. Joseph County (Vice Chairman 1999). Fellow, Indiana Bar Foundation.
